For prospective clinical, community & evaluation partners

Help turn pregnancy history
into completed care.

Make pregnancy history a lasting part of every woman’s heart care, from postpartum through midlife and menopause, so she understands her history, knows her next step and can reach the care she needs. Start with a conversation about what your organization can realistically support.

Founded by Lisa Zahakos, RPA-C. Proposed implementation begins with New Jersey. Clinical care remains with participating care teams.

Pregnancy history recorded.
Appropriate follow-up completed.

The first evidence of readiness is a willing team, a workable care pathway and an agreed way to measure completion.

Choose a contribution

Where could your team help?

01

Clinical host or reviewer

Review the education, identify eligible women and confirm the maternal-to-primary-care handoff. Name the team responsible for clinical decisions and responding to concerns.

Useful first contribution: a clinical contact and a description of an existing follow-up pathway.

Discuss clinical hosting →
02

Community or parent adviser

Help shape language, recruitment and support for barriers such as transportation, childcare and time away from the NICU. Include Black parent voices and families with different access needs.

Useful first contribution: a proposed community contact and a plan to resource participation.

Discuss community participation →
03

Evaluation partner

Agree on the baseline, denominators and confirmation of completed care. Assess feasibility, missing data, costs and differences in access through approved systems.

Useful first contribution: an evaluator and a brief assessment of the data the team could use.

Discuss evaluation →
04

Fiscal lead or funding adviser

Assess organizational eligibility, contracts, staffing and allowable costs. Confirm how program activity and clinical services would be paid for before recruitment.

Useful first contribution: an eligibility discussion and a review of the proposed budget.

Discuss funding or fiscal leadership →

A government or association contact can help by introducing an appropriate state, clinical or community lead. Risk-model research requires a separate protocol and team. Read the research scope →

Proposed 30-minute partner working session

A meeting with decisions
to take away.

Decide whether a local education and follow-up pilot is feasible, what is missing, and who will resolve it. A date and participants would be agreed after initial interest.

  1. 0–5 min

    Agree on the first population.

Choose a setting and a manageable group, such as postpartum women with pregnancy-related hypertension, including NICU mothers.

  2. 5–12 min

Walk through one existing care pathway.

Identify where the history is recorded, who gives the plan, who owns clinical follow-up and how completion can be confirmed.

  3. 12–20 min

    Confirm delivery and access.

Discuss clinical ownership, navigation time, community input, referral capacity, language needs and payment for indicated care.

  4. 20–26 min

    Define evaluation and resources.

Review the baseline, observation window, data permissions and planning budget. Identify the evaluator and potential fiscal lead.

  5. 26–30 min

    Assign the next decision.

Record a named owner and due date for each unresolved item. Agree whether to proceed to feasibility work, revise the scope or pause.

Before enrollment

What needs to be
confirmed in writing.

Lisa proposes to lead education, engagement and coordination. Each participating organization would approve its own contribution and accountable staff.

Budget, evidence & measurement definitions ↗
  • A responsible clinical team. Eligibility, review, referrals and escalation belong to named clinicians.
  • A usable care pathway. Appointments, monitoring when indicated, language support and payment are feasible for the women invited.
  • Resourced delivery. Staff time, paid community participation and organizational responsibilities are agreed.
  • An evaluation plan. Baseline, denominators, observation window, privacy and any quality-improvement or research review are settled.
  • A decision to launch. The fiscal and clinical leads approve the scope, budget and required agreements.

Partner interest is an early step. It does not establish an institutional endorsement, committed funding or patient enrollment.

Use Maya’s fictional story to review the history, visit questions, clinician-owned plan and follow-up status. Then try four short tasks and record what was clear or confusing.

This review tests understanding and usability. Clinical use still requires an agreed host, care pathway and local review.
