Clear to us.
Clear to her?

Help Lisa find out. Try the fictional example as a woman, family member, clinician or potential host. Tell us where the next step is clear and where the experience needs work.

Use Maya’s fictional story only. Please do not enter anyone’s name, contact details or health information.

Four things to try

Can you find
what matters?

Read each task, then try it in the example. A facilitator should observe first and offer help only after you have had a chance to try.

  1. Find the uncertainty.

What does Maya remember? What still needs checking?

  2. Find what she can take away.

Locate the sample Heart Story and the questions she can bring to her visit.

  3. Find the responsibility.

Identify the example action, responsible team, agreed date and access barrier.

  4. Tell the outcomes apart.

Try the three follow-up statuses. Which one counts as completed care?

Facilitator answer guide

1. High blood pressure and early delivery are patient-reported; the diagnosis, gestational week and discharge recommendations are unknown.

2. Step 2 links the sample PDF and shows four visit questions.

3. Step 3 names the record review and primary-care visit, a fictional primary-care team, November 18, 2026 and childcare-related scheduling needs.

4. Only confirmed completion counts as completed care in the example. An unknown status or a revised plan does not.

What happened on each task?

From feedback to a real pilot

The next decisions have owners.

1. Improve the experience

Lisa can invite an initial group of women, families and clinicians, record the tasks tried and revise the parts that cause confusion. Repeat the difficult tasks after changes.

2. Confirm a host

A willing clinical team must agree the pathway, staff, access support, data process and resources in writing before recruitment.

3. Measure real follow-up

The host and evaluator agree the baseline, definitions and observation window before collecting any results. Report missing data and barriers with successes.
